Eligibility
PTs and PTAs are eligible to receive the same award more than once. The highest frequency to receive the same award is once every five years. e.g., Jane Doe, PTA receives the 2030 PTA Award of Excellence. She will be eligible to receive the 2035 PTA Award of Excellence again. Awards that can only be received once include: Otto A. Cordero Lifetime Achievement, Hall of Fame, and Rookie of the Year.

Procedure For Nomination Of All Awards
1. Nominations may be submitted by any APTA Wisconsin member except for the Clinical Instructor of the Year which may be submitted by an Academic Coordinator of Clinical Education, a Clinical Instructor or a student.
2. All materials must be received in the Chapter office by January 31st for consideration for the award in the following year. Submitted materials will not be returned.
3. The nominator is responsible for submitting the following documents as a single packet via our online Google form:
Nomination form
Support statement from nominator
Optional: Two letters of support
Optional: CV of nominee
4. Current members of the Awards Committee shall not nominate; write letters of support, or endorse members for these awards.
Procedure for review:
1. The Awards Committee will review the nominations and recommend an appropriate recipient for approval.
2. The decision of the Awards Committee pertaining to the selection of the awardee will be final.
Notification of award:
1. The recipient of the award shall be notified promptly after selection by the Awards Committee
2. The nominators of individuals not selected will receive a letter thanking them for their participation
3. Awards will be presented at APTA Wisconsin Spring Conference by the Chapter President and members of the Awards Committee.
4. An appropriate announcement of the award will appear in PT Connections, APTA Wisconsin newsletter.
PLEASE NOTE: APTA Wisconsin respectfully requests that when noting receipt of an award, the year should be included in such recognition" e.g. "2014 APTA Wisconsin Hall of Fame Award". Alternately, if the award was received in a prior year it could be noted as "Past Recipient of
APTA Wisconsin Hall of Fame Award"
Nature of award:
1. A plaque will be presented to the awardee(s).
APTA Wisconsin Outstanding Service Award
Nomination Deadline: January 31
Purpose: To acknowledge an individual who has made significant contributions to the profession of physical therapy in Wisconsin as evidenced by distinguished and conspicuous contributions in the area of service to the Association and in the areas of education, research, or practice which have been marked by excellence.
Eligibility:
1. Nominee for the award must be a member of APTA Wisconsin in good standing.
2. Members of the Board of Directors and the Awards Committee are not eligible for nomination or consideration for this award.
3. Nominee must have been a member of APTA Wisconsin for at least ten years.
Criteria for Selection:
- Nominee must have rendered meritorious service to the APTA WI.
- Nominee must have done two of the following:
a. Performed teaching of notable quality in academia, the clinic, or as part of continuing education, or has shown exceptional leadership in the administration of an academic PT or PTA program.
b. Performed and published basic or applied research which furthers the science of physical therapy.
c. Demonstrated exceptional leadership in the management of PT services or delivery systems.
APTA Wisconsin Award for Clinical Excellence
Nomination Deadline: January 31
Purpose: To recognize an individual who has made significant contributions to the physical therapy profession in the area of clinical practice.
Eligibility:
1. Nominee for the award must be a member of APTA Wisconsin in good standing.
2. Members of the Board of Directors and the Awards Committee are not eligible for nomination or consideration for this award.
3. Nominee must have engaged in clinical practice for at least three years.
Criteria for selection: The recipient shall have made high quality contributions in one or more of the following clinical areas.
- Clinical practice
a. Direct patient care
b. Development of treatment procedure or protocol in a specific patient population
c. Development of critical pathways for specific patient populations
- Publication of clinical research
APTA Wisconsin Hall of Fame Award
Nomination Deadline: January 31
Purpose: To recognize an individual who has made lifelong contributions to the physical therapy profession as a member of the APTA WI.
Eligibility:
1. Nominees for the award must be life or life affiliate members of APTA Wisconsin (or meet criteria for same) in good standing.
2. Members of the Board of Directors and the Awards Committee are not eligible for nomination or consideration for this award.
3. Nominees must have been a member of APTA Wisconsin for at least ten years.
Criteria for selection: The recipient(s) shall have made contributions in at least one of the following areas:
1. Teaching in the clinic, academia, or continuing education
2. Leadership of an academic PT or PTA program
3. Delivery or administration of patient care
4. Publication of research
5. Service to APTA Wisconsin
APTA Wisconsin Clinical Instructor of the Year Award
Nomination Deadline: January 31
Purpose: To recognize an individual who has demonstrated excellence in the clinical education of physical therapist and/or physical therapist assistant students.
Eligibility:
1. Nominee must be a physical therapist or physical therapist assistant with a minimum of two years of experience as a clinical instructor.
2. Nominee must be a clinical instructor in a clinical center that affiliates with a Wisconsin PT or PTA educational program.
3. Nominee must be a member of APTA WI.
4. 1. Members of APTA Wisconsin Board of Directors and APTA Wisconsin Awards committee are not eligible for nomination or consideration for this award.
Criteria for selection:
1. The nominee's exceptional teaching effectiveness.
2. The nominee's enthusiasm for teaching.
3. The nominee's excellent communication skills
4. The nominee's excellence as a role model for students
Nominations may be submitted by an Academic Coordinator of Clinical Education, a Center Coordinator of Clinical Education, a Clinical Instructor, or a student.
APTA Wisconsin Mentor Award
Nomination Deadline: January 31
Purpose: To recognize an individual who has influenced and nurtured the development of PTs and/or PTAs in an outstanding or unique manner.
Eligibility:
1. Nominees for the award must be members of APTA Wisconsin in good standing.
2. Members of the Board of Directors and the Awards Committee are not eligible for nomination or consideration for this award.
3. Nominees must have positively affected the professional development of the PTs and PTAs with whom they are associated.
4. Nominees must have been actively engaged in mentoring activities for at least three years.
Criteria for selection:
1. . The nominees' effect on the professional development of other therapists
2. The nominees' excellence as a professional role model
APTA Wisconsin Physical Therapist Assistant Award of Excellence
Nomination Deadline: January 31
Purpose: To recognize a physical therapist assistant who has demonstrated outstanding achievement in clinical practice, community service and personal commitment to the field of physical therapy.
Eligibility:
1. Nominee for the award must be a member of APTA Wisconsin in good standing.
2. Members of the Board of Directors and the Awards Committee are not eligible for nomination or consideration for this award.
3. Nominee must have engaged in clinical practice for 3 years.
Criteria for selection: Outstanding achievement in the practice of physical therapy and consistent service to the profession beyond that of being a APTA Wisconsin member as demonstrated by:
1. Promoting the role of the physical therapist assistant through education, practice or research
2. Continuing professional development to optimally practice as a physical therapist assistant
3. Community service and activities that enhance quality of life for others
4. Service to APTA Wisconsin
Wisconsin Private Practice Award
Nomination Deadline: January 31
Awarded to a practice for innovation operation or a specific project which promotes a positive image of Physical Therapy in the community. Also acknowledges an individual member who has made outstanding and enduring contributions to the practice of Physical Therapy.
Eligibility:
1. Practice owner/director/manager must be a current member of APTA Wisconsin in good standing.
Criteria for Selection:
Must have positively affected the shape, scope and a quality of physical therapy private practice in the community and be consistent with the Wisconsin PPSIG values:
- We value business practices that promote the development of small business private practice.
- We value the promotion of reimbursement for services commensurate with physical therapy as a doctoring profession that is patient centric.
- We value collaboration and alliances among private practices that fosters growth, and continued advocacy of individual practices.
- We value business model innovation that continues to promote private practice physical therapy and physical therapy as a doctoring profession.
- We value our ability to work together; leveraging our group's purchasing volume to negotiate better rates on goods and services for the benefit of our members.
- We value direct access and the consumer's right to choose their physical therapist as their preferred provider of neuromuscularskeletal care, without barriers.

Rookie of the Year Award
Nomination Deadline: January 31
Purpose: To recognize an individual who has made significant contributions to the physical therapy profession in the areas of clinical practice or service within three years of graduation from an accredited PT or PTA program.
Eligibility:
1. Nominee for the award must be a member of APTA Wisconsin in good standing
2. Members of the Board of Directors and the Awards Committee are not eligible for nomination or consideration for this award.
3. Nominee must have graduated from an accredited PT or PTA program within the past three years.
Criteria for selection: The recipient shall have made notable contributions in one or more of the following areas:
1. Clinical practice
2. Publication of research
3. Service to APTA Wisconsin
Otto A. Cordero Award for Lifetime Achievement
Nomination Deadline: January 31
Purpose: To recognize an individual who has made sustained and significant contributions to the profession of physical therapy and to the Association over the course of their career.
Eligibility:
1. Nominee must be a APTA Wisconsin member in good standing.
2. Nominee must have been a member of APTA for 15 years or more.
Criteria for Selection:
1. The nominee's consistent service in a variety of voluntary, elected, or appointed positions within APTA Wisconsin or APTA.
2. The nominee's significant contributions to the profession or the Association while in these positions.
Nature of the Award:
1. A plaque will be presented to the awardee.
2. $500 shall be contributed to one of the following entities, at the discretion of the awardee, in the awardee's name.
a. WPT Fund
b. WPT-PAC
c. Foundation for Physical Therapy
APTA Wisconsin District Volunteer Service Award
Nomination Deadline: January 31
Purpose: To acknowledge an individual who has made significant contributions to the profession of physical therapy as evidenced by distinguished service to their district.
Eligibility:
1. Nominee for the award must be a member of APTA Wisconsin in good standing.
2. Nominee must have made substantial contributions to his/her district.
3. Nominee must have been a member of APTA Wisconsin for past 5 years.
4. Members of the Board of Directors of APTA Wisconsin and members of the Awards Committee are not eligible for nomination or consideration for this award.
Criteria for selection
- Nominee must have rendered meritorious service to his/her APTA Wisconsin district in one of the following ways:
- Demonstrated exceptional continuous engagement in the district for a minimum of 5 years.
- Demonstrated exceptional leadership in at the district level.
